The Mental Health Department is currently seeking a License Clinical Social Worker, Licensed Professional Clinical Counselor or Clinical Psychologist to provide therapy services in our Concentrated Mental Health Program (CMHP). This initiative focuses on providing comprehensive outpatient services to patients who have moderate mental illness including schizophrenia, bipolar disorder, PTSD, depression and other challenges. We are seeking team members who have experience with moderate to high acuity levels, co-occurring disorders, harm reduction, SUD and medication assistant treatment.

Duties and Responsibilities:

  • Conduct intakes with new patients and provide care during program participation
  • Create a comprehensive individualized treatment plan that includes goals, projected program exit date, and group assignments.
  • Coordinate with assigned primary care providers, case managers, and patient support team
  • Provide intensive weekly individual and group therapy to program participants
  • Address co-occurring disorders and coordinate with multidisciplinary team
  • Complete all required documentation accurately and timely
  • Upon completion of program, transfer patient to appropriate resources so they can continue care (most typically another FHCSD Mental Health Therapist)
  • Connect patients to other internal/external resources as appropriate
  • Performs other duties as assigned

Requirements:

  • Clinicians for the Concentrated Mental Health Program must have an active registration in California as a Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), or a Clinical Psychologist (PsyD.)
  • 2+ years of experience working with high acuity patients while fully licensed preferred

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
