DESCRIPTION

Under close supervision of the Care Management leadership, the Clinical Social Worker provides social work services consisting of discharge planning, continuing care resources and clinical social work services, including crisis intervention, patient advocacy mandated reporting, informational and referral coordination of health and mental health services with community providers. The Social Worker is responsible for education of patients, families and/or significant others. Participates in related department functions, such as community and program education.

The ED Social Worker collaborates with the interdisciplinary team, including but not limited to care managers, physicians, and nursing to identify social barriers preventing an alternative discharge strategy for those patients who do not meet acute level of care. The ED social worker provides appropriate community resources, referrals, and follow up care for patients to prevent unnecessary admissions or readmissions to the hospital and inappropriate utilization of the ED.

Depending on experience of the selected candidate, this position will be hired at the Clinical Social Worker I, II or III level.

MINIMUM QUALIFICATIONS
  • A Master's degree from an accredited school of social work (MSW).

  • Must possess the knowledge and abilities essential to the successful performance of the duties assigned to the position.

  • Excellent interpersonal and communication skills.

  • Must be able to work within a team environment.

PREFERRED QUALIFICATIONS
  • California LCSW license (must have this certification to be hired at level III).

  • Proven clinical training and/or work experience in the acute care, inpatient setting.

  • Experience working with specialty medical populations, transplant, oncology, NICU and/or psych.

SPECIAL CONDITIONS
  • Minimum two (2) years of post-MSW Social Work experience is required to be hired at level II.

  • The pay scale for the CSW1 (less than 2 years post-MSW experience) is $67,421-88,781. The pay scale for the CSW2 (2+ years post-MSW experience) is $82,747-$109,014. The pay scale for the CSW3 (with California LCSW) is $92,853-$122,315.

  • Must be able to work various hours and locations based on business needs.

  • Employment is subject to a criminal background check and pre-employment physical.

Pay Transparency Act

Annual Full Pay Range: $82,747 - $109,021 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $39.63 - $52.21

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
