Under the supervision and guidance of Managed Care and Social Services leadership team, The Clinical Coordinator provides clinical oversight of case management services within the assigned care management and social services program. Job duties include direct and indirect reports and providing clinical consult for assigned patient cases.
General Job Description:
Job Roles
- Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards. In addition, documentation complies with patients' health plan portal requirements.
- Works effectively as a member of the multi-disciplinary treatment team. Communicates with providers, physicians, case managers, scheduling staff, referring providers, and with outside agencies and authorities, in a timely and appropriate manner. Connects patients to internal/external resources as appropriate; accurately documents referrals.
- Acts as clinical case management resource for assigned staff.
- Provide clinical guidance for staff when completing health risk assessments, to include mental health, substance use disorder, chronic health conditions and social determinants of health.
- Provides clinical oversight of comprehensive, Individualized Care Plan (ICP), to include working with the patient and their care team, based on the patient's health status, needs, preferences and goals.
- Reviews for appropriateness and signs referrals for Community Supports when licensure clinician review required by the health plan.
- Supports Interdisciplinary Team calls with health plans and community providers for CalAIM (California Advancing and Innovating MediCal).
- Reviews charts to provides appropriate coordination of care and delivery of all needed ECM services, consults with program leadership to address any services that do not meet standard of care.
- Supports department with case management and documentation standards.
- Acts as subject matter expert regarding staff development and training practices. May conduct mock assessments with newly hired case managers.
- Reviews, cosigns, and ensures accuracy of clinical assessments leading to the creation of the patient's individualized plan of care. Coaches staff on assessments and care plans.
- Provides timely consultation within clinical scope for care managers when a client is experiencing a crisis.
- May directly supervise a small team.
- Builds positive relationships with staff and inspires others to perform at a higher level. Meets regularly with staff and compiles individual weekly/monthly reports to encourage staff performance.
- Develops and maintains positive relationships with appropriate referral agencies and other community and social supports.
- Leads weekly case consultation group meeting for unlicensed department staff.
- Actively participates in agency, division, and department meetings; maintains continuing educational and professional licensure as appropriate.
- Performs other duties as assigned.
Education/Certifications/Licenses/Registrations
- Current Clinical licensure required: Registered Nurse, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, Licensed Clinical Psychologist, etc.
- Meets organizational driving requirements.
- LCSW, LMFT and Licensed Clinical Psychologist candidates to be licensed and practicing for at least 2 of the last 5 years, have completed or willing to complete clinical supervisory training within 30 days of hire, have practiced psychotherapy during at least two (2) years out of the last five (5) years OR provided direct supervision to ASWs, Associate Professional Clinical Counselors, Marriage and Family Therapist Trainees, or Associate Marriage and Family Therapists who perform psychotherapy during at least two (2) years out of the last five (5) years preferred.
Experience/Specialized skills (including Language)
- Exceptional verbal, written and listening skills.
- Strong analytical and problem-solving skills.
- Ability to work with vulnerable populations and coordinate care for those clinically complex.
- Experience in building coalitions/collaborations preferred.
- Successful track record of leading and driving positive change through others.

$100,000.00 - $125,987.00If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
