Community Support Program Nurse (RN)

Pay Range: $74,000-$84,000
Sign-On Bonus: $3,000.00

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time RN for our Community Support Program (CSP). CSP nurses work as a part of an interdisciplinary treatment team providing services to adults with severe and persistent mental illness.
Responsibilities
- Conduct comprehensive nursing assessments as part of a multi-disciplinary treatment team – initial health screenings, follow-up nursing appointments, various testing
- Consults with Riverbend providers, team members, family members, significant others, and community providers.
- Maintain your own caseload and cover other caseloads as needed
- Medication management – provide client education, administer medication (long acting injectable), refill management, prior authorizations, consults with pharmacies
- Attends treatment team meetings and other meetings as required
- Collaborate with Riverbend Integrated Center for Health (embedded PCP practice)
- Utilize electronic medical record (EHR) to document patient information, referrals or other relevant information as required.
- Adhere to all compliance procedures relevant to protected health information (PHI) and HIPAA regulations.
Requirements
- Valid New Hampshire Registered Nurse License
- At least one-year nursing experience required
- Certification in Basic Life Support
- Experience working with individuals with psychiatric illnesses preferred
- ANCC board certification in Psychiatric Mental Health Nursing preferred
- Valid driver’s license and personal vehicle or access to a reasonable alternative is preferred
